RIDER APPLICATIONS If you would like to apply to ride at The Shelley Centre please telephone 01473 824172 or email theshelleycentre@aol.com to arrange for an application form to be sent to you. You will need to leave your name and address, the name, age and weight of the rider, and details of their disability. This application form must be completed by the rider or their parent/guardian, together with their Doctor, Physiotherapist or other health professional. As riding takes place during school hours it is advisable to discuss the possibility of having time out of school to participate in this therapy, with the school authority,  before completing the application.
